The Visiting Nurse Association of Mercer County coordinates the patient’s home health care with physicians, referral sources, insurance companies and family members. We are an extension of your physician’s recovery or care program. Our experienced home health care professionals successfully combine skill with compassion and care.

Skilled Nursing

VNA nurses provide in-home care and instructional guidance in caring for the patient. They work closely with the patient, the family and the physician in preparing a plan of care. Visits are scheduled based upon the health care needs of the patient.

Our nurses are experts in all home health care areas, including geriatric care; post-surgical care; post-stroke care; wound care; maternal-child health; rehabilitation; the chronically ill; terminal care; home infusion therapy.

Competency testing is a continuous process to assure our professionals maintain the highest skill and knowledge levels.


Medical Social Services

Certified social workers assist clients and their family with non-physical problems, including the emotional and financial impacts of illness; referrals to community resources; financial entitlement.

They counsel and assist individuals and families with coping skills; mental health issues; support issues; abuse/neglect issues; safety concerns; financial concerns; housing options; grief therapy and long-range planning assistance.


Home Health Aides

The Home Health Aide, is an important member of the Visiting Nurse Association, is trained to assist patients with a variety of tasks under the supervision and direction of the patient's nurse. Services may include baths, preparation of meals, change of bed linens, light household chores, and monitoring of temperature, pulse and respiration.


Rehabilitation

Rehabilitation encompasses: occupational therapy, speech therapy and physical therapy.

  • Occupational Therapy: The VNA Occupational Therapist cares for the patient with upper extremity limitations, illness, or injury that interferes with ability to perform activities of daily living or homemaking skills by providing equipment and training.
  • Speech Therapy: The speech therapist works with a patient recovering from a stroke or other condition affecting speech and language. The patient is taught basic speech/language patterns and when necessary alternative modes of communication.
  • Physical Therapy: The physical therapist cares for patients recuperating from strokes, joint replacements, bone fractures and other similar conditions. The Therapist makes arrangements for proper equipment and provides instruction for use of the equipment. In addition, the patient and family are taught safety and proper techniques necessary for everyday living within the home environment.
