Seeking to provide progressive, compassionate home and community health services to all those in need, the Visiting Nurse Association tries to meet the increased medical needs of clients through specialty programs. With increased health care being provided in the home, Visiting Nurse Association seeks to offer care and services related to nutrition, diabetes, prevention of falls and wound care. Each of these additional services is delivered by a trained professional who is part of the Visiting Nurse Association multi-disciplinary team of health care providers.

Diabetes Management

A service delivered by our visiting nurses, in conjunction with our nutritionist/certified diabetic educator. Patients are taught the essentials of managing their diabetes, including medication compliance, skin care, diet teaching, blood sugar monitoring and appropriate patient response to a diabetic emergency.


Prevention of Falls

Falls prevention is a major health, social, and economic concern within the health care industry. Falls can cause disability or even death. Falls are the leading cause of injury deaths for Americans over the age of 65 and account for over 70% of accidental deaths for people over the age of 75. Falls can be prevented.

The Visiting Nurse Association of Mercer County has developed a three-phase innovative program designed to educate patients, caregivers and health care providers; provide education and outreach to the community; and, assess older patient needs within the home environment.

Prevention of falls by VNA will be addressed through literature, seminars, individualized assessment and instruction.


Wound Care

The VNA professional meets with the patient and evaluates the necessary course of care for the wound. The wound care specialist works with insurance companies and vendors to offer the patient the latest technologies and products to improve wound healing.


Maternal/Child Health

A service provided primarily to well mothers and newborns, usually within the first ten days of birth. Specially trained nurses are able to evaluate both patients for any deviations from the norm. The visit includes health education regarding the care of the infant and an assessment of the mother's physical status post child-birth.
