The VNA is proud to be the home for the Medicaid Waiver Care Management Unit for Mercer County. The state of NJ has designated the VNA to provide care management for the following state funded programs; Global Options (GO) for Long-Term Care, Jersey Assistance for Community Caregiving (JACC), Community Resources for People with Disabilities (CRPD), Aids Community Care Alternatives Program (ACCAP), and the HIV Homecare Program.

These programs offer access to a wide range of in-home services to seniors and adults with disabilities who meet income, asset and nursing facility level of care criteria established by Medicaid. The Care Managers work closely with participants and their caregivers to identify needs and barriers that put individuals at risk for placement in long term care. An individualized plan for care is developed, implemented and monitored by the Care Manager to ensure quality and effectiveness.

The primary mission for the Care Management Department is to provide supportive services for those in need while ensuring their dignity and independence in the setting of their choice.
