Hospice and Palliative care is provided by an interdisciplinary team of experienced and caring professionals including your physician, the VNA Hospice Medical Director, Registered Nurses, Medical Social Workers, Spiritual Counselors and Certified Home Health Aides. Highly trained volunteers provide a variety of services that are tailored to the needs and wishes of each patient. VNA Hospice can also arrange for Physical, Occupational, or Speech Therapy, and a Registered Dietician to address nutritional needs.
Treatment plans are individualized to address the unique aspects of the patient's care, and updated regularly as their condition changes. Our staff teach patients and their caregivers how to alleviate and control symptoms with medications or other therapies, and to manage the care that will be needed to remain at home. Medical equipment and supplies are sometimes provided to promote comfort and safety making day to day care easier. A nursing manager is available to problem solve with patients and families 24 hours a day, 7 days a week to achieve effective pain and symptom management. If needed, our Registered Nurses are available after hours to make visits for more complex problems that cannot be resolved with telephone consultation.
Paying for Hospice Care
Hospice care is a covered benefit under Medicare and Medicaid. In addition, most private health insurance companies and many HMOs cover hospice as the benefits of palliative care become more and more evident. Coverage generally includes home visits by members of the hospice team, as well as medications, medical equipment and supplies related to the terminal condition.
