The VNA employs a Certified Diabetic Educator/Nutritionist to make home visits as needed, provide patient consultation, and collaborates with our Registered Nurses to develop an individualized plan of care.

Patients are taught the essentials of managing their diabetes, including medication compliance, skin care, diet teaching, blood sugar monitoring and appropriate patient response to a diabetic emergency.
